Unless you ask us not to, we may contact you via email in the future to tell you about specials, new products or services, or changes to Guelph MMA policies.

The undersigned swears that the provided information is true and accurately reflects the status of the students’ current information.  It is understood that this information is provided for the students own safety and authorize Guelph MMA to keep a copy of this document for their records. 

The undersigned herby declares that they are physically fit to participate in the prescribed course of instruction.

The undersigned understands that during the course of instruction, employees of Guelph MMA and/or other students or authorized personnel will be engaged in a course of conduct requiring physical contact and fully consent to such contact as required by the training. 

The undersigned understands and hereby fully releases Guelph MMA, its shareholders, directors, officers, employees, those for which Guelph MMA is responsible at law or otherwise, the Landlord of the premises and any other party or parties related to any of the foregoing persons from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the Member’s use or intended use of the facilities and training, including but without limitation, any claims for personal injury arising from or as a result of the negligence of Guelph MMA, its owners, agents or employees or the negligence of any other person present on Guelph MMA premises as members.

Guelph MMA reserves the right to alter the days and/or hours of operation, the curriculum and the amount of classes you're entitled to attend, if deemed necessary. Guelph MMA does not guarantee that you will reach a certain level of competence within any specific time frame and that students' obtaining a certain defined level of competence may be either delayed or accelerated due to changes in the curriculum or as deemed necessary by Guelph MMA. The student and co-signer agree unconditionally to the above without regard to the attendance.

Any pictures, videos or written documentation may be used for promotional purposes without financial compensation payable to the student or otherwise.
